We are Geriatric Care Managers. As elder care specialists, we assess seniors in their home or facility, then determine a plan of action to help them receive the care they need. We create and implement a care plan according to the responsible parties’ wishes. We are advocates, and help sort through all of the options available. We act as liaison with physicians, attorneys, home health agencies, home care agencies, care facilities, and hospices, etc.
